Palliative Care

Dr. Kirk Hamilton is a palliative medicine FRCPC specialist who has focused his practice on managing palliative care home based patients (caring for patients with a life limiting illness in their home environment).

Dr. Hamilton is the lead physician of the London Home Palliative Care (LHPC) team and this physician team helps to manage 110-150 home based patients that are living with an end stage illness. We are available to manage acute patient issues with our home based palliative care patients 24/7. The majority of our patients (over 86%) either remain at home for the duration of their end of life care or have had an arranged (facilitated) transfer to a palliative care unit without an unplanned admission to one of our acute care hospital beds.

Our LHPC team physicians are experienced and skilled in managing our palliative care home based patients.

Dr. Hamilton often has doctors who are in their residency programs on the LHPC team who are passionate about learning how to manage palliative care patients under Dr. Hamilton’s guidance and supervision.

The number of palliative care home patients that this LHPC team is able to care for is limited, based on the current roster of patients that the team physicians are able to safely manage at that point in time.
